    Immunizations

    • Tetanus
    • HepA HepB
    • Pneumovax
    • Flu Shot

    Screening

    • Pap Test (>21 q3y if normal, later if not sexually active)
    • Mammography (>50 q2-3y)
    • Colon Cancer (>50)
    • Bone Mineral Density (>65)
    • Eye exam
    • Depression - PHQ2
      •  In the past two weeks, have you felt down or depressed?
      • In the past two weeks, are you enjoying things as usual?
    • Cardiovascular
      • Fasting Blood Work

     

    Safety

    • Sun safety
    • Calcium (1200mg)
    • Vitamin D (800-1000mg)
    • Exercise: 30min of moderate intensity exercise 5x/week (need to get heart rate up)
    • Diet: Usually discussed by dietician or nurse just prior.

     

    Functional Inquiry

    Physical exam (female)

            normal Skin: no rashes, no abnormal nevi
            normal HEENT: PERL, fundi N, no thyromegaly, carotids N
            normal CVS: normal S1/S2, no S3/S4, no murmurs
            normal Resp: good a/e bilaterally, no crackles, wheezes
            normal Breast: no skin change, no masses, no adenopathy
            normal Abdo: soft, non-tender, no masses, no organomegaly

     

    Physical Exam
